Jeremy Mattern founded The Mattern Foundation with a deep commitment to transforming the lives of gay men in recovery. Driven by a vision for meaningful community impact, Jeremy has dedicated himself to creating pathways for those who are too often overlooked.

Before joining the Foundation, he founded Ed Fusion Strategies, a consulting firm dedicated to helping school systems develop and implement program strategy to increase postsecondary outcomes.
He also served as Program Officer at Houston Endowment, where he led a portfolio of education grants and mission-driven initiatives. He is a Licensed Social Worker who has spent much of his career helping individuals and organizations overcome barriers to success.

Jeremy is the President of The Mattern Foundation, which provides support and resources for gay men overcoming challenges with substance abuse and addiction. Before joining the Foundation, he founded Ed Fusion Strategies, a consulting firm dedicated to helping school systems develop and implement program strategy to increase postsecondary outcomes.
Jeremy also served as Program Officer at Houston Endowment, where he led a portfolio of education grants totaling $20M annually. He has also worked at a leading education technology firm that partners with higher education institutions to use technology solutions for achieving their institutional goals and strategic priorities. He previously served as the Director of College Success at the Houston Independent School District, where he and his team led district-wide college counseling and advising efforts, increasing equity and access to postsecondary opportunities.
He is a Licensed Social Worker who has spent much of his career helping individuals and organizations overcome barriers to success. At home in Houston, you will find him enjoying coffee (or wine) on the back porch with his partner and two long-haired Dachshunds, or traveling to spend quality time with his two nephews.
Dr. Nicholas Crapser is currently the Director of Behavioral Health and Recovery Services at 4D Recovery. Dr. Crapser is a State Licensed and Nationally Board-Certified Mental Health Counselor, state and nationally certified substance use disorder counselor and clinical supervisor. His education is in chemical dependency studies, developmental psychology, and counseling with doctoral and post-doctoral education in counselor education and supervision, forensic psychology, and positive psychology. He has specializations in neurofeedback and dialectic behavioral therapy and has received advanced training from the University of Cincinnati Corrections Institute — School of Criminal Justice. He has worked in each modality of drug treatment and mental health from medically managed care/withdrawal management to aftercare services. He spent six years as the Deputy Director of Sponsors, Inc. in Lane County, Oregon, bringing mental health treatment to the organization and helping to greatly enhance existing programs and housing to individuals released from incarceration.
Before 4D Recovery, Dr. Crapser worked for the Oregon Department of Corrections as the Behavioral Health Services Director overseeing mental health treatment delivered both to inpatient and outpatient housing at three of Oregon’s Correctional Institutions. While at the Penitentiary, Nick started a Peer Recovery Mentor program, licensing adults in custody as Certified Recovery Mentors (CRM) and as Certified Alcohol and Drug Counselors (CADC) starting a Diversion Program based on a Drug Court Model allowing adults in custody the chance to choose diversion as opposed to punitive measures for failing drug tests.
In addition to working in the field, Nick is also an adjunct professor teaching on the subjects of addiction, mental health, justice involved populations (both youth and adults), involuntary commitment, and reentry. He has been an associate and adjunct professor at Oregon State University, Rutgers University, Lower Columbia College, and Lane Community College and has been invited as a guest lecturer at numerous other universities. Nick’s passions are helping youth, those justice involved, formerly incarcerated, and those suffering from mental health and substance use disorders — subjects that he has spoken on from coast-to-coast including TEDx Humboldt State University, Rutgers University, Columbia University, Arizona State University, and speaking events in Washington, D.C., Boston, Florida, and California. He is on the board of the Lane County Public Defenders Office and has spent years on the Lane Community College Board and steering committee. He is the immediate past Vice President (Northwest Region) of NAADAC, the National Association of Addiction Professionals. In 2019 he was awarded the Community Leadership Award from Serenity Lane and in 2022 the Innovator of the Year Award from ORAAP. In 2023, he was awarded the “Excellence in Addiction Medicine” award from the Oregon Society of Addiction Medicine.
Brad is the treasurer and a board member of The Mattern Foundation, which provides support and resources for gay men overcoming challenges with substance abuse and addiction. Before joining the Foundation, he was the president of a non-profit focused on harm reduction and overdose management at large LGBTQIA+ festivals. He also was a division chair and is currently a committeeman for the Houston Livestock Show and Rodeo Sports Medicine Committee, which is a volunteer organization for the world’s largest rodeo and livestock show that offers full-service preventative care, urgent care, and emergency medical treatment for the rodeo contestants and their families.
Brad is a board-certified emergency medicine physician who practiced clinically before transitioning to health insurance and now works for Aetna as a senior medical director with a special focus on gender affirming care. He serves as a member of the Pride Committee and is a champion for LGBTQIA+ causes. Since moving to the health insurance realm, he has successfully advocated for expansion of fertility benefits and gender affirming services to include all members of the community.
Preventing harm and providing necessary treatment services while promoting positivity and erasing stigma from and toward the community are Brad’s driving forces in every endeavor. In his spare time, he keeps busy with his husband and pets (dog and cats), plays with his ever-growing number of nieces and nephews, and maintains self-care through yoga, meditation, and getting immersed in TV.
Colt has a degree in Public Administration from St. Edward’s University. He is a prior Congressional Capitol Hill Intern in the United States Senate, Presidential Freedom Scholar, and Senior Counselor at American Legion’s Boys State Program.
Colt is an avid outdoorsman, traveler and played competitive rugby internationally in both Rome, Italy and Amsterdam Bingham Cups. Colt also grew up on a rice farm in Arkansas and is a proud uncle to 9 nieces and nephews.
Derek became a Nurse Practitioner in 2016, has been a RN for over 15 years and has been with the Crofoot team for 10 years. He began his career in healthcare as a Medical Assistant in the ER in Ohio where he grew up. His first job as a RN was at Duke University’s Medical Center in the CT Step Down unit while enrolled in Duke’s Nurse Practitioner program with a sub-specialty in HIV care.
After growing up in Ohio, Derek attended Bowling Green State University for undergrad and Duke University for grad school. He takes pride in creating a safe and welcoming environment for the LGBTQ+ community to address their unique healthcare needs. In his free time, you will find Derek feeding his Crumbl Cookies addiction and mapping out ways to advance his life’s mission of achieving the title of Taylor Swift’s #1 fan.
David currently works with his brothers-in-law as the Director of Operations at WJM Custom Builders LLC after a 25-year career as a commercial litigation attorney in Houston. David also earned his Master of Divinity from Fuller Theological Seminary and is ordained within the PC(USA).
At home, David enjoys spending time with his wife and three daughters. Together, they love on their dog, Jake.
Doctor Shannon Schrader is a board-certified family physician who has been practicing medicine for over 30 years. Originally from Kentucky, he attended the University of Kentucky for his undergraduate degree and the University of Louisville School of Medicine to complete his medical education. His internship and residency brought him to Houston, where he completed his training at Baylor College of Medicine. He was honored to be chosen to serve as Chief Resident during the last year of his training.
A big part of Dr. Schrader’s professional career has included working with HIV, which has been a bridge into caring for and embracing the LGBT+ community. As a result, he has encountered many aspects of the LGBT+ family. Treating the physical complaint of a patient may involve more than the chief complaint. Dr. Schrader understands that a physical presentation may get someone into the office, but his role is to also explore the mental, spiritual, and emotional aspects of a patient’s history and life. Often, this may be an issue with addiction. For 23 years, Dr. Schrader volunteered at Legacy Clinic (formerly Montrose Clinic). In addition to his private practice, he currently serves as Chief Medical Officer of Triangle Area Network (TAN) in Beaumont, Texas. TAN clinic is a Federally Qualified Health Clinic that provides care for uninsured, homeless, and the communities of Beaumont and Orange, Texas. He often states that his own emotional tank is replenished after visits with the clinic’s patients.
Furthermore, Dr. Schrader is the Chief Medical Officer for the J Flowers Health Institute. J Flowers provides a comprehensive approach to exploring the mental and physical health of clients who are struggling with complex medical diagnoses and uses a team of experts to help overcome and conquer their problems. In his spare time, one will find Dr. Schrader reading an “edgy” novel, eating popcorn while enjoying a movie, or traveling to his home on the coast of Maine. He also says that his faith is his driving force from within that carries him through each day. Dr. Schrader considers his faith the seat belt that holds him steady when life can present itself as a bumpy ride.
As an M.D. with advanced understanding of clinical medicine and science, Rayan offers subject matter expertise in psychiatry and a portfolio balancing clinical, educational, research, and administrative accomplishments.
His knowledge exceeds psychiatry and focuses on the interface between psychiatry and other medical specialties in general, and specifically in neurology. Both colleagues and residents consider him a psychopharmacology expert, particularly in the areas of mood disorders and psychosis. He has a strong understanding of medicine, mechanism of action, and side effect profiles and management.
During residency, Rayan joined the Research-Educator Track with Dr. Lauren Marangell. Since early residency, he has been involved in many research protocols and part of design teams. Rayan has been part of single-site research, multicenter, and global research serving as principal or co-investigator on 30+ research protocols with diverse funding sources, including NIH, VA, foundations, industry, and local support.
